With United Healthcare EPO in MS, what is needed to get approved for wls?

My insurance with United Healthcare EPO in MS will become effective on Feb. 1st and I would like to know how long it usually takes for them to approve you for surgery and also what do they require to get approved. I don't really have any co-morbidities, but I do have a BMI in the 50s. Thanks for any help you can offer me.
