Cigna member with questionable co-morbidity and BMI less than 40. Will they approve?

I am a Cigna member with OA of both knees well documented in my medical record. However my BMI is only 38. I have no other health issues that are significant to meet Cigna's criteria other than OA in knees. Anyone know how tough they are on co-morbidities with low BMIs?
