Letter of Medical Necessity from Plastic Surgeon

It has been my experience that few plastic surgeons can write a successful letter of medical necessity. I have the typical physical problems associated with massive weight loss and large pannis. I have accumulated a wealth of information regarding medical necessity and related this to my plastic surgeon. I also supplied a copy of a position paper in hopes that he could write a suitable letter to acquire insurance approval. My insurance company has denied coverage based on his letter and my plastic surgeon refuses to submit a letter of appeal. I am in search of a doctor that is versed in writing successful letters of medical necessity. I am in the Kentucky-Tennessee area. Any information or ideas would be appreciated greatly.
