How should I go about having a revision done?

I am now 4 years post op and would like to have a revision done. My beginning weight was 364 and I now weigh 257. I do realize that the surgery is only a weight loss tool. I believe my pouch was very big from the beginning. I have had a scope done at it shows everything in tact. I'm interested in The Sapala-Wood Micropouch Roux-en-Y. I began with a Normal Roux-en-Y. Where do I go from here?
