What is the procedure for approval in VA if I have BCBS/Carefirst of MD?

I currently live in Virginia but I have BCBS/Carefirst. Will my claim go to the local office (Trigon) or will it go to the MD division. Would the VA or MD law regarding obesity surgery apply?? Are they easy to deal with??
