Nutrionist eval

Does anyone know what is needed in the nutrional eval. My nut is getting on my nerves and I feel like she is holding me back by trying to find out a lot of information that is not necessary (trying to find a reason to not be able to approve me). Every other doctor I went to they had a brief questionaire and then wrote a letter, she on the other hand seems to have pages and pages of questions that she has pulled offline. Like wtf. So any help is appreciated...I am getting desperate!
