back pain

I have been having so much upper back pain. The pain is due to my breast. How do I get my onsurance to pay to have a lift. I would also like to have a lower body lift. But my insurance says I would have to have sores. I am a very clean person. I bath two times a day. I have no sores because I do keep myself clean. So how can I get the insurance to cover it.
