Do anyone know what the approval process for EHP insurance is.

I have emailed surgeon for consult. appt. I am still waiting for a response. However, I am aware that he will have to submit a letter for approval to the insurance company. I am trying to figure out what is the criteria for approval from EHP. Any help to this question will be greatly appreciated,
