I need to fill out FMLA paperwork but wanted to have my PCP fill out, not the surgeon

The company I work for is bad about keeping peoples info confidential even if you request this so I do not want the weight loss surgeon to fill out my FMLA papers because I am a nurse & work in a facility where her name is known so word would get out what type of surgery I am having, but my PCP says that he cannot fill them out because he does not know the info. I am not embarrassed in any way about the surgery-there are just people at work that I don't care for them to know my business. Any ideas?? Thank you!
