Need advice on a persuasive letter to my PCP

I AM TRYING TO FIND OUT WHAT I AM SUPPOSE TO WRITE TO MY PCP TO BETTER MY CHANCES FOR APPROVAL FOR THE SURGERY SHE APPEARS TO BE AGAINST IT SHE IS A CPT AND SHE HAS PUT IT IN THE NUTRITIONIST HANDS SO WHEN THE NUTRITIONIST SAYS OK I CAN GO BACK TO SEE THE SURGEON SO NOW THAT I HAVE FOUND OUT THAT I DO NOT HAVE TO GO THROUGH ALL OF THAT WHEN I VISIT HER ON THE 19TH I WOULD LIKE TO INCLUDE A LETTER WITH MY VISIT SO CAN ANYONE GIVE ME SOME POINTERS THANKS AHEAD FOR THE SUPPORT
