Has anyone had problems getting their PCP to sign a medically supervised diet form?

I have Aetna QPOS and my PCP refuses to sign it. This is required for insurance approval and I am at a loss as to how to proceed. I am so close, I have everything BUT this. Any advice or anything would be appreciated. You can post here or email me at [email protected]. Thanks.
