Everyone is dragging their feet, what should I do??

I had my consult with my surgeon on June 28th and assumed the letter would be sent out within the next few days. I called my insurance company a week later and found out they hadn't received it, when I called the doctor's office the receptionist said she was busy and wouldn't be able to get it out for about another week and a half!!! It's going on over two weeks and she said she'll have it out by the 16th "for sure." The problem is my company may be closing in the next month and I want to get this pushed through my insurance ASAP! What should I do??
