What Happens Now

I have had all of my pre-op testing, even on an nutritional program after the surgery. What happens now, do the surgeon calls the insurance company and get approval or do he just see what date will be available for surgery. I called his office and asked his secretary, but they are all so busy and just keep telling me we'll let you know. Also isn't there a time limit from the testing to the actual surgery.
