Has anyone ever had a pre-existing clause lifted from their BC/BS policy?

I have spoke to a supervisor and he thinks I should grieve my situation. So I faxed a letter. They will not DENY or APPROVE my request for pre-approval for the surgery. I need help on how to go about this. The hospital will not go head without pre-approval for payment from insurance co.
