Please Read if you Belong to a Self-Insured Group

If you belong to a self-insured group, please know they are exempt from state regulations governed by the insurance commissioner's offfice. However, they are governed federally by the Department of Labor under ERISA. The web address is www.dol.gov. You can find information there on how to appeal, how to get more definitive information about denials, etc. Thought I would pass this on in my fight for reconstructive plastic surgery. Good luck.
