Anyone have experience with state of Michigan retirees BCBS PPO?

I have very limited documented info about weight loss records, and I have listed everything I can remember that I've done in losing weight. I don't know if I need to try to get documents from doctors from 5+ and some 10+ years ago or if they'll take some of it on my own word. Are doctor's readily agreeable to give you your records (even if they get paid for copies) for this surgery or doesn't it matter that they even know? Thanks for any info anyone out there has. Thanks
