ANYONE WILLING TO SHARE SAMPLES OF INSURANCE APPROVAL LETTERS

i am in the begining steps of the process, my pcp supports me in the choice for the Lap bypass, my surgeon and she feel i would make a good candidate. i think i'm nervous though as far as what to write to my insurance company. i dont have many co-morbs, so my main "leg to stand on" is the fact that i'm 100+ over weight and a bmi over 40. any advice???? all info and feedback will be greatly appreciated thanks jennifer
