NGS American Insurance-What is their approval rate?

I have done everything they have asked including the 6 month supervised diet. I am waitng on preapproval and it seems that the ins. company has always come back wanting more information. After 1 1/2 years it now sounds now as if they have everything they need and I should know in 7-10 days. Has anyone out there been denied by NGS and what what was the reason for denial.
