Can anyone help with a sample weight loss documentation letter

My ins. co. is requesting a 6-month dr. supervised diet document and I've researched and haven't found any sample letters. A few have offered but their profiles are no longer available. Can anyone help? I have an appointment with my PCP tomorrow to go over this and I need to be prepared. Thank you in advance. Janet P
