Surgeon requiring medical history...

My surgeon is requiring a 2-year medical history as part of the package to send to the insurance for approval. Due to either not being employed and not having any insurance or being employed at a company that didn't offer insurance, there was a period of about 6 years I didn't even go to the doctor. I didn't get insurance until May of 2003 and have been regularly seeing a doctor since. Obviously, this isn't the 2-year requirement. Has anyone had a similiar situation and how did you gracefully explain it to the surgeon or the insurance company? Thanks for any help.
