State of Indiana Employees - MPlan

Has anyone had approval recently with MPlan? If so what kind of hoops did you have to go through. I am considering surgery and need to know where to start. I am 270#, 5'3" with non-insulin diabetes and have records for a diabetic diet for the last 3 years so am technically qualified. Do I need to see my PCP or find a surgeon first? Any help or advice is appreciated! Thanks, DeeLyn
