Has anyone gotten approval in WA state w/medicaid?

Does anyone know what the requirements are for approval w/medicaid in WA? Or where I can get the info? Also, what doctors accept medicaid for gastric bypass? Any info on how easy/difficult it was for approval, waiting time, the whole process in general is appreciated!
