HELP PLEASE i was denied TT coverage from select blue of Western Pa

Would anyone be willing to share their letters they sent in for approval? Any help would be greatly appreciated. They said the way my surgeon sent in the request made it sound as for cosmetic reasons only! The surgeon said they couldnt send any other info in except what they already sent.
