Does anyone have BC/BS of Alabama with 20/80 plan?

If so, what was your copay? Did your surgeon accept the insurances paymment, I know they have to but I am wondering if it causes a problem? Were there any hidden fees? Can you please contact me via email, I have suddenly become afraid of copays!
