Looking for a Psychologist in Arizona- Pacificare HMO Friendly

HAS ANYONE IN AZ HAD PACIFICARE HMO COVER THEIR PSYCH EVALUATION, IF SO, NAME OF DR? I am trying to find the name of any doctors in az that are contracted with pacificare hmo that does the mmpi psych evaluation. I have been going round and round trying to find out who I need to see and keep hitting a brick wall. :)
