I have pushed my Dr. for 2 months to get my Pre cert letter done. Not yet!

I have bugged the office every couple weeks to push him along. He is a wonderful Dr. and I love him dearly but how do I get him to write the letter. I even made an appointment a month ago and gave him alittle cheat sheet(a summary of my health history) I thought this would help so he wouldn't have to did through my chart. HELP!!!!!
