Need Help With Surgeon Support Letter for Plastics

I am writing an email to my WLS surgeon, requesting a letter of support from him that I may send to my insurance company while seeking approval for my abdominoplasty and body lift. But, what exactly should I be asking him to say? What role would a letter from him play? Any help would be appreciated. Thanks!
