Looking for WLS friendly PCP in San Jose, CA who accepts UHC

My surgeons office has given me several names of WLS friendly PCPs but they are a bit far for me, though I will try one of them if need be. I was hoping to try closer to home first, also realizng they will need to be In-Network for me. Thanks! Lynn
