Anthem covering lap-band?

I have a friend in Columbus who is seeking Lap-band surgery. She has Anthem insurance. Anthem will pay surgeon fee but not hospitalization, anesthesia, etc etc at OSU in Columbus (hospital doesn't "take" anthem i guess). Anyone know of surgery completely covered (except copay of course?) for lap-band? She is willing to travel for surgery. Thanks!
