Anyone have Cigna PPO and get approved for a tummy tuck?

What is the criteria for getting approval from Cigna PPO for a tummy tuck? I'm looking into possibly having a tummy tuck. Would a tummy tuck be fine for me? I have very poor muscles in the abdominal area due to 2 pregnancies and carrying the babies high. Neither ever dropped and I have always had a belly. My back hurts a lot (lower back) and I sweat bad in that area. I could also be about 2 sizes smaller if I didn't have the pouch. Crunches and sit ups never helped me. I've never had a "flat" belly.
