What are out of pocket costs for fobi w/Dr. Husted?

I am fairly certain my insurance (UHC, POS)will cover my surgery, but I am concerned about out of pocket costs. I understand Dr. Husted requires a "deposit" (refundable after ins. pays)prior to surgery. I am just concerned-how much am I going to have to spend prior to surgery. And what if I get denied after paying for office visits, etc.?
