Does anyone know how much you have to pay out of pocket for Kaiser of CA (Bay Area)?

I have a friend who would like to know how much the cost of surgery is with Kaiser of California/Bay Area. Also, for those of you who got Kaiser to pay, was it difficult? Did you have to appeal? What should my friend do to get the ball rolling to try and get Kaiser to approve her WLS? What percentage does Kaiser usually pay for WLS?
