Hello I saw a plastic surgeon, what can I do for a denial?

Hi, I saw a plastic surgeon a few mos ago, and today I was told that my insurance United healthcare denied my request for the Mammaplasty surgery. I am 10 mos. post-op and have lost -142lbs. I am having a huge problem with back pain from my breasts and Apron. What do I do now?
