What do I need to include in my 2nd appeal, which is a hearing with insurance reps?

I sent out my first letter of appeal and was denied. In this letter I included all my health issues and why I need to have this surgery. Now they have scheduled a metting for the second appeal, what should I take with me to this meeting?
