Any post-ops out there who had surgery and then moved?

I had my open RNY in November 2001. I may be moving in the next few months and was wondering what to do about follow up. Do I need to find a surgeon to follow me or does my surgeon help me with that? Also, how does another surgeon feel about taking on somebody else's patient? If anybody out there has gone through this please let me know. Thanks!
