My insurance is changing from Humana to Aetna what do I do for pre-existing condition

My employer is changing insirance companies from Humana PPO to Aetna PSO (Prudential) how do I fill out the pre-existing condition section of the paperwork? I have no surgery date but have been to a consultation w/ a surgeon. Can I just list I have been seen by my PCP and a specialist?
