I don't think that the VSG people in the US have to do anything like that but i am sure your doc has good reasons! Try posting on the VSG board.

For the Brazil option, ask the US forums (main board) - they are geographically nearer and lots seem to go to Mexico for surgery.
Is aftercare an issue for the DS? For lapbanders, not being near your doc for the year post-op would be a big negative. I understand that extensive lab work and blood tests are on-going for DS patients and you would need to make sure you can get those done here if you don't get surgery here.
