Try looking up armchair exercises or arthritis exercises on line. There are ideas for things that don't involve sore joints. Try water walking, the water supports the knees. Use soup cans, veggie cans for upper body workouts, progress to milk jugs for increased weights. Any exercise helps, stationary bike, for range of motion for the knees.
First thing is wrap your mind around the idea that "I can do things" and find the things that do not aggravate your knees. Maybe even some physical therapy for the knees. Mine were were sore last winter, I was to the point of using a cane. I did go to physical therapy along with my supervised diet, the treatments made my knees feel better, before long, the pain was under control enough that I could do some walking and left the cane behind. The combination of diet and exercise is the key.
You can do this, I am here rooting for you. Hugs, Kathy

Kathy gave you most excellent advice. I agree with her suggestions 100%! She's a smart cookie!
Now for my 2 cents.
I would suggest that you try to follow a diabetic diet. This is much like how you will eeat post-op. But also try having 5 very small meals a day, instead of 3 larger ones. This tells your body the food is always coming and it is less resistant to letting go of the stored fat. Be sure to get in breakfast within 30 minutes of waking up, as this will help set a higher metabolism for the day which promotes fat burning as well.
Look into the exercises Kathy recommended, as when it comes down to it, it is calories in versus calories out. Even the upper body work will add up in calories spent.
The other suggestion is to talk to your surgeon. Let him know how hard you are trying and that you are working with the nutritionist and that you fear not being able to lose the 40 lbs. See what he has to say.
Keep us posted on how it goes. Don't give up. Just try a new approach!
Hugs and love to you!
