Plastic Surgery? Will it be needed?
I'm afraid it's a your-mileage-may-vary thing. I didn't need it but others with similar starting BMIs did.

Like Gina said, it varies and depends on many factors. I am having a TT this fall. I lost over 100 lbs so not comparable to you and have an apron of skin from hip bone to hip bone. (YES I have hip bones now, lol).

Age, genetics, body build, where you carry the fat, all play a part. I certainly could use some but at 64, I'm not going there. I don't have any rubs or anything that would get insurance to cover it. And I look pretty good with clothes on.

Like others have said - it all depends.
I think age and how long you were heavy has a lot to do with it. I was 49 when I had my surgery and did have a TT and BL/BA done about 1-2 years later.

Regain 30 lbs from 2012 to 2016 - got back on track and lost it. Took 8 months.
90+/- pounds lost BMI - 24 or so

I only lost 65 pounds but my double chin turned into a turkey neck so I had a lower facelift one year after WLS. And my tummy, after a pregnancy and the big weight loss, looks like a shar pei. I don't have a pannus but it's wrinkled and awful looking, with little "love handles" that won't go away because they are just skin. So I have booked a tummy tuck for August.
I think there's really no way to know what you'll need until you need it, or not.
