I have a feeling that if you lost the other 17 lbs, he would refuse to do the surgery on you then because your BMI is lower... and he'll say "see - you can do this yourself"... wouldn't surprise me a bit.

Go check out the other surgeon that doesn't require pre-op diets and waiting periods.  Get your info transferred.  It will be a delay, but the frustration and stress won't be there for you...
