High PTH

I just got my labs back and everything looks good except my hemoglobin and hematocrit, which have always been on the lower side of normal (even though my Ferritin and other iron numbers are good) and my PTH is too high for my liking.  What is strange is my D is pretty good.  It was 64 in Aug. unless it has tanked since then.  What's funny is back when my D was barely undetectable my PTH never got above 50.  Last January it was 20 and now it is 63.  What is up with that?  My calcium was 9.6 last April.  My labs are out of sequence since I have different tests run at different times depending on what my PCP wants.
I do worry about my D since back in January it dropped 27 points in 6 days for some reason.  On January 13 it was 78 and on January 19 it was 51 then in August it was 64.  I want to ask to get my D tested again but I know I would be in for a fight because it was just done in August and my doctor doesn't think there is anything to worry about as long as the numbers are in the normal range no matter how close to the top they may be.
Of course I have increased my D I am just wondering what else could cause a high PTH if not low D?

How are your hormones? I was put on hormone replacements to help bring my PTH, D and calcium in line. They also put me on Fosamax at that time to address my osteoporosis.

I can't get a straight answer on my hormones.  Whenever I request any kind of hormone testing I am told that they aren't valid because hormones flucuate so much during the course of a day.  I am talking about estrogen and other female hormones.  I was told not to take hormone replacement unless I have problems with hot flashes as that is really all they help.  I do use estrogen cream.
I do wonder if this has anything to do with the Reclast infusion I had in May?
I should contact my endo.  Maybe I need to have my D checked again.

You seem to have a lot of fluctuations in your labs. Are you sure your lab is competent? I know my mom went to a place that totally screwed up her liver biopsy and she'd always had problems with them so that was the last straw. She changed labs and hasn't had issues ever since.

I have wondered that myself.  It is a large teaching university hospital so I assumed they were competent but I wonder.  Maybe they have students running the labs?  Maybe I just have a strange body.

Oh teaching hospitals are known for eratic standards of care!

I had my pulmonary embolism at a teaching hospital and while most of the care was good, I once I to correct a nurse who was teaching other nurses how to change the IV bag who turned my drip OFF instead of back ON. And I was half asleep because it was the middle of the night. I'm sure she just got confused but still...

When my D3 was higher than they wanted, like 128, my PTH was low.  When they took me off the D3 for a few months, it came down and the PTH came up but not to normal.  Now my D3 is lower than I want, 57.  I don't really understand the relationship between those and calcuim but they were ok with PTH being low, just not high.  So, all that was just to say, I don't know but I'm hoping yours gets straightened out.  I do know that  I feel better when my D3 is higher.

You want the PTH to be low (in the 20's or low 30's is better), calcium around 9.2 and D at the high end of the range or just a little higher.

My D could be higher but the NP has a fit if it gets above 80.  She told me she never had a WLS patient with a D level as high as mine when it was 87.  She worries about toxicity.  She is thrilled where it is now and wants me to continue doing what I am doing but I am upping it one more pill a week.  My calcium is pretty good, too.  I know they worry about it getting too high because that is a sign that it is being leached from the bones because we aren't getting enough.  I just don't know how high is considered too high.  I'm just a little worried about the PTH because there are other reasons for it to be elevated besides not enough D and calcium.  I'm going to send my endo a message and see what he says and if it's related to my osteoporosis or it's treatment.
