My surgeon does six months, 1 year and then yearly.  But, because my primary knows I have other concerns (cholesterol, thyroid, etc) he wants to do them also.  So now I alternate and get labs every six months, once from primary and once from surgeon.  My primary is pretty good about adding any tests I'm concerned about.

My surgeon checks labs at 6 months, 12 months, 18 months, 24 months, then annually.  My primary care doc checked some selected ones (iron, thiamine, etc) at 8 weeks out, then he also does some selected ones at that annual appt.
