Tips for initial consult?
I wouldn't fast. I would eat everything I could right before my appt so I would weigh in as heavy as possible. If you need more bloodwork you can always come back another day for it. Also ask your treating physician to write a letter of support regarding your GERD and Barrett's esophagus. I'm sure your surgeon will guide you as well. Good luck!
Here's another question:
Should I be concerned about losing too much weight on "medically supervised diet?" Do I want to make sure to stay over BMI of 35, or does it not matter? I read in some forum that their docs prescribed phentermine during the supervised diet. I'm pretty sure I could drop about 20 lbs on phentermine and WW, but I'd hate to be sitting there at 215 with a bmi of 33 or whatever and out of the running for surgery.

Try not to lose too much during the supervised diet. It's an insurance hurdle only. ASBMS has come out saying it provides no benefit to the patient.
