You've been on my mind since I read this note.
I went to my support group last night and someone asked the question about weight loss slowing/stalls. The doctor gave a really good answer. I don't know if it will help, but I thought I'd share. First let me say that my surgeon’s practice consists of several surgeons and an internal medicine doctor who is the clinic director. She is EXTENSIVELY trained in bariatric medicine. She is flabbergasted with bariatric practices that recommend their patients eat 5-6 small meals a day. The research is very clear that, in those people whose metabolism has slowed, you need calorie deficit periods during the day. You can’t get these by snacking or eating 5-6 times a day. You get these periods by eating 3 meals a day and having 4-6 hours between meals where you aren’t eating anything….ANYTHING. Your first few months out, the focus should be on be getting in all your protein and water. You really shouldn’t worry about your caloric intake. If you only eat 3 meals a day, your small pouch won’t allow you to eat too many calories. Your body will use the energy it eats before it uses the energy it stores. If you eat throughout the day your body will adjust and learn to survive on the calories you are feeding it. With the deficit periods, there is no energy from food and the body must take fuel from your fat stores. Eat your protein first and avoid simple carbs like bread and sugar. Simple carbs are fairly easy for your body to process. Your body will use that energy instead of your fat stores. (If you are diabetic, there are another slew of issues with simple carbs.) The #1 reason people with RNY regain their weight is snacking. My doctor wants us to establish long term eating habits of 3 meals a day…NO SNACKS! Personally, I was off track. I feel like my losing has slowed. I added a snack in the morning and the afternoon. They were low calorie, but snacks anyway. I was also eating simple carbs after my protein. For example, tuna and crackers seem to settle easier than just tuna. This meeting really highlighted what I was doing wrong and helped me refocus my efforts. Stacey, I hope this helps. One of the other WLS patients asked the same follow-up question. Her answer was yes. If there are calories in your protein shake then that should be considered a meal.
Many of us still drink some of our protein. She understands, but says that should be considered a meal replacement. She recommends we eat our protein whenever possible because you stay satisfied longer.
Once you get further out (past 6 months or so) and your pouch can hold a little more, she says only eat when you are hungry. I'm still at the stage where all my hunger is head hunger.
Also, everyone has different caloric needs. Don't use averages or what other people are doing.
Frankly, with 3 meals a day, protein first, I am pretty low in calories. I have a shake for breakfast. My pouch is grouchy in the morning, and my shake is only 100 calories.

This is so interesting. My plan has been to eat 3x a day, 4-6 hours apart, all food without supplementation since surgery. I love reading this explanation because it makes so much sense about the "resting" periods. Thanks so much for taking the time to post this! :)
