Very nice!
Your PCP might say your B-12 is high. Smile and nod but keep doing what you're doing.
I see no tests for D3 or A. Tell her you won't leave until she gives you a script for vitamins D3 and A. They are fat soluble and you malabsorb some fat so these need to be checked. She might not even get that your duodenum is bypassed and that's where D and A are absorbed in normies but not us.
Keep working on the iron and ferritin. They're not bad at all, pretty good in fact, but you might be able to do a little bit better.
Your PTH and Calcium aren't bad but you need to see D3 to go with them to really tell how you're doing.
AST and ALT are very nice. Liver function is good.
But, these are pretty nice labs. Rock on!

Do you need the link to Vitalady's suggested labs?
All you can do is try with the new PCP. Can you shop around if you get one that's not WLS friendly?

Here's the link. Most docs don't want to order all these but see if you can find one willing. Good luck!
