update
ok, so my NUT was not able to change the dates which means i dnt have it all complete exactly for 90 days. I am submiting the papers as is and hoping they dont notice or complain , if they do then i will hhave to pospone surgery and go through it all alone cause my mom cant take another week off. 
God grant me with the serenity to accept the things i cannot change ; Courage to change the things that i can ; and the wisdom to know the difference!!!
