Do you mean the psych eval?
Mine was very simple and no, I've never met anyone who failed it.
It seems that they wanted to make sure you were looking to have surgery of your own choice (not being forced by someone) and had realistic expectations about the surgery, and are capable of complying with the rules afterwards.
