Lab Test Request--

I'm looking over the lab testing request form that was given to me at my first appointment. I didn't even think to ask at the time what a lot of this means.

I'm wondering if anyone else was given something like this that lists certain blood tests to be done after having that taken, what they want with the x-rays, etc. There are numbers beside these descriptions underneath the title "ICD9 Code/Diagnosis." The numbers are mostly in the hundreds and include decimals. I know this is one of the stranger inquiries to make, but these numbers are for the hospital's use, not fees, right?

I hope someone might know what I'm talking about, lol... Thanks in advance for any insight. :)

Yes, those are the diagnostic codes that the doctors offices use for insurance purposes, you don't have to use them. One less thing to worry about!
