I was 95% of the time you have to submit the 6month requiered diet but I was the 5% that didn't.
..all I need was a min of 5 years showing morbid obesty and by that I mean I had to submit the actual doctor's office/progress notes that had my weight written on it. I was told bascially that if I didnt have that I would definatley get denied. I have been at this since June so take your time and be persistant. Go online and sign up at united healthcare and then you can look up your benefits and coverage - special services...see what it says. Mine says that I must contact United Resource Networks...I did that in June and they expalined everything to me and the nurse there was very honest and helpful and actaully the person that approved me...Hopefully your company uses their service and then I can tell you it wont be that bad...the customer service people arent the best...so my advice is to start with the united healthcare website and take it from there...the surgical coordinator just swore up and down I need the 6th month diet because as she said she has not seen it not required...so good luck to you!!
