Hi Jamie,  I was required to have a 5 year diet history... I had no idea what dates and how much i lost and gained, so I told my surgeons office.  They told me that is really common and to just write my best estimate of each thing.  So, I did and I got approved. Best of luck to you.

OIh I am sorry, I guess I misunderstood.  Anyway, I would assume that is the same because its 5 years of diets and weights.  At least that is how I did mine anyway.   I was required to be at or within (+or - 40 lbs) for at least 5 years.

I would guess that they look into the weight history.  I have had the same Insurance all my life so if they really wanted five years over weight all they would have to do is look at my records, since I would weigh in at every appointment.  They never asked me to prove it though.

All insurance have their own sets of requirements which vary depending on how the contracts are negotiated with the employer your insurance is through. A five year history of a BMI of over 40 or sometimes 35 with comorbidities, is a common requirement. My plan (UnitedHealthcare) required it. I had to call my old doctor in AZ and ask them for my highest weigh in each of the last five calender years. They accepted the information I provided as long as I gave them the doctor's contact info. so that they could verify it if they wanted to.

I had to prove back 5 years of being obese.  My doctors office suggested I do that with my last 5 years of medical records, since EVERY time you go to the doctor you get weighed in, so all those weights would prove that I had been above obses for at least 5 years.  I also had to do a 1 year diet requirement, thankfully that was met with my 16months of Weigh****chers records. But like Michael said EVERY insurance is different.

Thanks everyone! The thing is I haven't been to the doctor every year for five years. It would be back since I was 16. So hopefully they don't require it. I am going to call today and get a straight answer. Thaks! ~JAMIE~
